PRN Registered Nurse
About The Position
As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will help promote client independence, ensure safety, and enhance quality of life for clients and their families. The role involves providing direct care and nursing services in client homes, documenting care, collaborating on individualized service plans, and supporting clients with practical household needs. The position requires daily travel to client homes and occasional visits to the agency office. Scheduling is flexible and may include weekends and holidays, with compensation provided for those visits.
Requirements
- Current, valid RN or LPN/LVN license.
- Reliable personal vehicle with valid insurance.
- Clear criminal background check.
- Negative pre-employment drug screening.
- Strong verbal and written English communication skills.
- Ability to lift at least 25 pounds.
Responsibilities
- Provide personal care, including bathing, dressing, grooming, oral care, and hygiene support.
- Assist clients with ambulation, wheelchair use, and safe transfers.
- Support toileting needs, including catheter and ostomy care.
- Reposition bedridden clients and maintain clean, comfortable bedding.
- Assist with meal preparation and nutritional intake as needed.
- Monitor and record vital signs and document intake and output.
- Administer and manage medications as prescribed.
- Assess clients’ nursing needs on an ongoing basis and provide nursing services according to established care plans.
- Report changes in condition, concerns, and progress promptly to the Case Manager and physician.
- Complete accurate and timely clinical documentation.
- Participate in developing and implementing individualized service plans.
- Observe and report changes in clients’ mood, behavior, or emotional status.
- Communicate effectively with interdisciplinary team members.
- Perform light housekeeping, including laundry, vacuuming, dusting, and kitchen tasks, to maintain a safe and healthy environment.
- Organize client living spaces to promote safety and comfort.
- Assist with errands and transportation, including medication pickup and grocery shopping.
- Practice within the legal scope of licensure and maintain professional appearance and boundaries.
- Promote teamwork, unity, and respect, and adapt to additional responsibilities assigned by leadership.
