About The Position

Albuquerque Health Care for the Homeless (AHCH) is seeking an engaged and energetic Registered Nurse (RN) to join their diverse provider team. This role is primarily at the 24/7 Gateway Medical Respite Center, with some time spent at the downtown clinic for meetings and limited shifts. The RN will provide professional nursing care to patients in the medical clinic and on outreach within the Bernalillo County service area. This position requires collaboration with other cross-disciplinary staff on physical and mental health issues, and adherence to OSHA and CDC guidelines. AHCH is dedicated to serving individuals experiencing homelessness and believes in solving homelessness through healthcare, housing, and a living wage.

Requirements

  • Ability to thrive in a complex healthcare setting
  • Strong desire to work in an integrated/cross-disciplinary team model
  • Experience documenting services in an EHR
  • Knowledge of diagnostic and treatment room procedures
  • Experience with health promotion, education, and maintenance
  • Knowledge of medications and their effects/side effects on patients
  • Ability to assess problems quickly and accurately and find appropriate solutions
  • Excellent communication skills and ability to educate clients on health-related topics
  • Ability to work with individuals from a wide variety of backgrounds and life experiences in a respectful and culturally competent manner
  • Graduate of a School of Nursing program
  • Current RN license in New Mexico or eligibility to obtain RN license in New Mexico
  • Experience working in a primary or urgent care clinical setting

Nice To Haves

  • Bachelor’s Degree of Science in Nursing (BSN)
  • 2 years of experience working in a primary or urgent care clinical setting
  • Previous experience with outreach/street medicine
  • Clinical experience with underserved populations, those with SMI/CMI, and substance abuse disorder
  • Previous experience working in a FQHC or community health setting
  • Bilingual Spanish/English
  • Certified in wound care specialty

Responsibilities

  • Work with other disciplines toward integrative approaches to patient care
  • Establish a compassionate environment by providing biopsychosocial support to patients
  • Manage the care of patients in collaboration with other health professionals and transdisciplinary teams
  • Serve as a cross-department clinical resource for AHCH staff
  • Collaborate with other staff in treatment planning
  • Participate in planning, case review and quality assurance/quality improvement activities
  • Participate in peer review process
  • Coordinate continuity of care for AHCH patients hospitalized and through medical respite upon release
  • Maintain standards of medical care according to established protocols
  • Ensure timely documentation of services in EHR in compliance with AHCH standards
