About The Position

Albuquerque Health Care for the Homeless (AHCH) is seeking an engaged and energetic registered nurse to join their diverse provider team. This role is for the 24/7 Gateway Medical Respite Center. The registered nurse will provide professional nursing care for patients in the medical clinic and on outreach in the Bernalillo County service area. The RN will also consult and collaborate with other cross-disciplinary staff on issues relating to the physical and mental health of patients. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. They are looking for someone who has worked in a family practice/primary care setting and has experience working in a street outreach or community health settings. The candidate should be adept in screening and prioritizing clients and have general experience in all types of wound care. AHCH complies with all OSHA and CDC guidelines regarding PPE and has various procedures in place to safeguard employees.

Requirements

Ability to thrive in a complex healthcare setting
Have a strong desire to work in an integrated/cross-disciplinary team model
Experience documenting services in an EHR
Knowledge of diagnostic and treatment room procedures
Experience with health promotion, education, and maintenance
Knowledge of medications and their effects/side effects on patients
Ability to assess problems quickly and accurately and find appropriate solutions
Must have excellent communication skills and have ability to educate clients on health-related topics
Must be able to work with individuals from a wide variety of backgrounds and life experiences in a respectful and culturally competent manner
Graduate of a School of Nursing program
Current RN license in New Mexico or eligibility to obtain RN license in New Mexico
Experience working in a primary or urgent care clinical setting

Nice To Haves

Bachelor’s Degree of Science in Nursing (BSN)
2 years of experience working in a primary or urgent care clinical setting
Previous experience with outreach/street medicine
Clinical experience with underserved populations, those with SMI/CMI, and substance abuse disorder
Previous experience working in a FQHC or community health setting
Bilingual Spanish/English
Certified in wound care specialty

Responsibilities

Work with other disciplines toward integrative approaches to patient care
Establish a compassionate environment by providing biopsychosocial support to patients
Manage the care of patients in collaboration with other health professionals and transdisciplinary teams
Serve as a cross-department clinical resource for AHCH staff
Collaborate with other staff in treatment planning
Participate in planning, case review and quality assurance/quality improvement activities
Participate in peer review process
Coordinate continuity of care for AHCH patients hospitalized and through medical respite upon release
Maintain standards of medical care according to established protocols
Ensure timely documentation of services in EHR in compliance with AHCH standards
