About The Position

Deliver comprehensive direct nursing care on a high-acuity medical-surgical unit by executing physician orders, implementing evidence-based care plans, and precisely monitoring and documenting acute changes in patient conditions. Serve as the primary clinical coordinator across multidisciplinary healthcare teams to streamline communication, manage complex treatment plans, and ensure well-coordinated, high-quality patient care. Proactively assess, manage, and coordinate comprehensive patient discharge planning needs from admission to transition, collaborating closely with case management and the healthcare team to optimize outcomes.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
