About The Position

The Registered Nurse (Clinic RN) serves as a care coordinator for patients throughout their cancer journey. The role assesses patients’ physical, psychological, and social needs and supports seamless care coordination, education, and patient support. The RN partners closely with providers to enhance patient experiences and outcomes through proactive communication and clinical excellence. The position is based at Wolf River and is listed as PRN.

Requirements

Graduate of an accredited nursing program.
At least one year of nursing experience in a medical, surgical, or clinical setting.
Current RN license in the state of employment or a compact license.
BLS certification is required for all new hires and transfers.
Knowledge of health system resources and patient navigation practices.
Strong written and verbal communication skills.
Conflict-resolution and problem-solving ability.
Proficiency in computer systems and documentation.
Compassionate and professional demeanor in patient interactions.
Commitment to excellent customer service and patient care.

Nice To Haves

Bachelor’s degree in nursing.
Experience in outpatient nursing and scheduling.

Responsibilities

Assess patients’ physical, emotional, and social needs to optimize care planning.
Coordinate communication between patients and providers to support continuity of care.
Guide patients through the healthcare system, ensuring timely access to education, treatment, and resources.
Track and document patient data accurately and in an organized manner.
Deliver direct patient care and perform clinical procedures within the scope of training.
Provide education and support to patients and their families throughout the care continuum.
Conduct telephone follow-ups and patient communication within the RN scope of practice.
Collaborate with multidisciplinary team members to ensure a comprehensive approach to care.
Perform other duties as assigned.
