About The Position
Registered Nurse (RN) – Home Health Assessments (Part-Time, Flexible Schedule) Location: Milford, MA (with travel to surrounding cities) Pay: $45–$50 per hour About Us We are a growing home health agency based in Milford, MA, dedicated to providing high-quality, compassionate care to clients in their homes and in assisted living settings. We are looking for a motivated and reliable Registered Nurse to join our team on a part-time basis, with the potential to grow into a full-time role. Position Overview This role focuses on conducting initial client assessments prior to the start of home care services. The RN will evaluate client needs, document care plans, and clearly outline diagnoses to ensure proper support and continuity of care. In addition, this role includes light community outreach and relationship-building with local facilities.
Requirements
- Active Registered Nurse (RN) license (Massachusetts)
- Strong assessment and documentation skills
- Comfortable using technology (iPad, electronic documentation systems)
- Excellent communication and organizational skills
- Ability and willingness to travel locally
- Self-motivated and able to work independently
Nice To Haves
- Prior experience and RN or LPN license required.
Responsibilities
- Conduct initial in-home or facility-based client assessments
- Develop and document individualized care plans and client diagnoses
- Accurately complete all documentation using our electronic system (Life Profile) on an iPad
- Communicate client needs effectively with internal care teams
- Travel to client locations within approximately a 45-minute radius of Milford, MA
- Perform light marketing/outreach, including: Delivering brochures to assisted living facilities and local partners, Building relationships with facility staff to support referral partnerships, Promoting services such as respite care and one-on-one support
