Requirements

  • Active and unrestricted New York State Registered Nurse (RN) license required.
  • Graduate of an accredited School of Nursing (Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)).
  • Current BLS (Basic Life Support) certification required.
  • Knowledge of New York State nursing regulations, clinical standards, and patient safety practices.
  • Experience working with Electronic Medical Records (EMR/EHR), preferably eClinicalWorks (eCW), Epic, or similar systems.

Nice To Haves

  • ACLS certification preferred, depending on specialty and practice setting.
  • Minimum of 1-2 years of nursing experience preferred; ambulatory care, primary care, specialty care, home care, care management, or outpatient experience strongly preferred.

Responsibilities

  • Conduct comprehensive patient assessments, including medical history, vital signs, and clinical evaluations.
  • Develop, implement, and update individualized nursing care plans based on patient needs and provider recommendations.
  • Administer medications, injections, treatments, and vaccinations in accordance with physician orders and nursing standards.
  • Monitor patient conditions, identify changes in health status, and communicate findings to providers and care team members.
  • Provide patient and family education regarding diagnoses, medications, treatment plans, disease management, and preventive care.
  • Coordinate care with physicians, nurse practitioners, specialists, case managers, and ancillary staff to ensure continuity of care.
  • Triage patient concerns, phone calls, and urgent clinical issues, providing appropriate interventions and follow-up.
  • Assist providers with examinations, procedures, and clinical documentation as needed.
  • Maintain accurate, timely, and complete documentation in the electronic medical record (EMR).
  • Review laboratory, imaging, and diagnostic results and facilitate appropriate follow-up care.
  • Ensure compliance with infection control practices, patient safety standards, HIPAA regulations, and organizational policies.
  • Participate in quality improvement initiatives, audits, and performance improvement activities.
  • Support care management efforts, including chronic disease management, preventive screenings, and patient outreach.
  • Respond to medical emergencies and provide appropriate nursing interventions within scope of practice.
  • Maintain current knowledge of nursing best practices, clinical guidelines, and regulatory requirements.
