About The Position

We are looking for a compassionate Registered Nurse to join a growing team and support clients in their homes. The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide home-based support. This is a PRN position involving local travel and a flexible schedule. Requirements

Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing.
Current state license as an RN.
Proof of current CPR certification.
Compassionate, friendly demeanor.
Excellent communication skills.
Current driver’s license and reliable transportation.
Ability to work a flexible schedule and travel locally.

Responsibilities

Visit patients in their homes for one-on-one visits.
Complete initial and ongoing assessments to identify home health patients’ physical, psychosocial, and environmental needs.
Assess patients’ conditions, chart observations, and perform evaluation tasks, including taking vital signs and reviewing medications.
Educate patients and their families on home health care strategies and procedures, and recommend devices or tools that could improve patients’ quality of life.
Communicate effectively with patients or clients, family members, team members, and other health care professionals.
