This position is for a Registered Nurse at the Paces Ferry Outpatient Clinic, operating on a PRN (as needed) basis. The role involves providing nursing and case management/disease management services to clients and families to aid in the coordination and management of their health needs across the full continuum of care. The Registered Nurse will assess, coordinate, plan, monitor, implement, and evaluate the patient's plan of care to achieve optimal quality, clinical, and financial outcomes. This includes performing assessments of clinical and life issues for patients, and providing professional nursing services in support of physicians in the direct and indirect care of patients. The position requires flexibility to work PRN between Monday-Friday, 8:00 AM - 5:00 PM, and attendance at orientation and initial onboarding for several weeks at the Newnan campus. The nurse will support the hospital's mission and vision by aligning actions with the hospital's goals, assessing patients' clinical and life issues, coordinating care plans, and collaborating with the interdisciplinary healthcare team. Expertise in oncology, chemotherapy, and its side effects is expected, including monitoring symptoms, educating patients and families, and addressing quality of life issues. Timely and thorough documentation in the electronic medical record, effective communication with the healthcare team and patients, and providing patient education are crucial. The nurse will also serve as a resource and facilitator, promoting communication and collaboration, and ensuring excellent customer service. Tasks will be performed according to facility policies, regulatory requirements, and best nursing practices, including central line care, medication administration, and procedure assistance. The role also involves efficient service delivery by covering for team members, responding promptly to patient needs, and meeting hospital turnaround time goals. Overall, the registered nurse plays a pivotal role in delivering high-quality, patient-centered care and supporting patients and families through their healthcare journey.
