About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This initiative aims to reduce the burden on patients who are transported for dialysis three times a week and to decrease avoidable healthcare costs. Teammates in this role will have access to DaVita's extensive capabilities and resources to develop and grow this new service line. The ideal candidate will have experience in dialysis, thrive in a start-up environment, be passionate about improving patient lives, and possess a proven ability to achieve results. Comfort working autonomously and collaborating with cross-functional teams is essential. In this position, you will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment is on track. You will troubleshoot and make sound decisions regarding dialysis treatments, occasionally partnering with SNF staff for care coordination. The primary work schedule is three days a week, with potential for occasional Saturday work. No on-call duties are required. Your work will directly impact the lives of patients and their families daily.

Requirements

  • Previous Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months' of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Basic computer skills and proficiency in MS Word and Outlook

  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians who are tasked with delivering tandem care to meet program capacity and efficiency goals.
  • Provide comprehensive patient care in an independent setting.
  • Assess, diagnose, and implement complete care plans by looking at the whole patient.
  • Manage and track patient care effectively.
