About The Position

The Registered Nurse will oversee, monitor, and coordinate the medical needs of adults residing in Residential Group Homes in Westchester, Rockland, and the Bronx. Most residents are dually diagnosed with a Developmental Disability and Mental Health Diagnosis. The role involves functioning as a liaison between medical providers and the clinical team, coordinating, monitoring, supervising, and implementing routine and preventative medical and psychiatric care. This includes attending medical, dental, and psychiatry appointments, conducting weekly visits to residences to review documentation and ensure follow-up, verifying MD orders are implemented, preparing clinical notes, and performing physical assessments as needed. The nurse will also attend monthly clinical meetings to ensure optimal medical/behavioral/psychiatric care, certify Direct Care Staff in medication administration, and provide education on relevant medical issues. Additional nursing-related duties may be required.

Requirements

  • Registered Professional Nurse in the State of New York
  • Experience working with Developmentally Disabled and/or Mental Health population is a plus.
  • Must be organized, enthusiastic, and compassionate.
  • Strong written and verbal communication skills.
  • Ability to work independently.
  • Strong clinical assessment skills.
  • Must be able to use a computer and learn software programs.
  • Must be able to operate a fax, copier, scanner, and other office equipment.
  • Must be able to speak, read, and write fluently in the English language.
  • Must be able to write in the English language using correct spelling, grammar, and punctuation.
  • May frequently lift 0-25 lbs., occasionally lift 25-50 lbs., and rarely lift over 50 lbs.
  • May frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-45 lbs. force, and rarely push/pull over 45 lbs.
  • Will be required to sit, stand, and walk throughout the work day.
  • May occasionally stoop, kneel, or crouch.

Responsibilities

  • Perform physical assessments on residents as necessary.
  • Attend monthly clinical meetings and assist the clinical team in providing optimal care.
  • Certify Direct Care Staff in medication administration according to OPWDD regulations.
  • Provide staff education and training on pertinent medical issues.
  • Perform additional nursing-related duties as required.
