About The Position

This individual is responsible for the daily clinical aspect functioning to meet all residents’ needs. The role involves medication supervision/administration, resident admissions, completing care plans, and potentially clinical system tracking for wounds or infection control under the direction of the Director of Nursing. The position also includes providing health consultations, notifying physicians and families of resident health changes, responding to emergencies, maintaining inter-departmental communication, and ensuring resident confidentiality. Educational meetings for residents and staff are also part of the responsibilities.

Requirements

  • An Associate’s Degree or equivalent from a two year college or technical school, or Two years related experience and/or training equivalent combination of education and applicable experience.
  • 1+ years of experience in a skilled, sub-acute, or memory care setting.
  • Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing.
  • CPR certified.
  • Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.
  • Basic computer skills.
  • Ability to work as part of a team and handle multiple tasks safely and effectively.

Responsibilities

  • Medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
  • Admissions of new residents to include all admission assessments and documentation.
  • Completing acute and long-term care plans or service plans.
  • Clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing.
  • Discharges, assessments, and clinical documentation as indicated.
  • Provide general consultation regarding health-related concerns within the community.
  • Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
  • Respond quickly to all emergencies.
  • Maintain close communication with all departments and staff members.
  • Maintain confidentiality of residents’ and prospective residents’ information.
  • Understand role in the Safety & Disaster Plan.
  • Provide educational meetings for residents and staff.
