About The Position

Alvita Care is seeking experienced, CT-licensed Registered Nurses (RNs) to provide private duty nursing services in Connecticut, specifically in Fairfield, New Haven, and Litchfield County. The ideal candidates are reliable, compassionate, and willing to deliver consistent, one-on-one care. This role offers per diem shifts ranging from 6 to 12 hours, with both day and night shifts available. Opportunities include skilled nursing visits and providing 1:1 care in the home environment. This position allows for part-time work, per diem, or scheduling around an existing work schedule.

Requirements

  • CT RN licensure.
  • BLS, ACLS, or CPR certification.
  • Minimum of One (1) year demonstrated experience as an RN.
  • Ability to put patients at ease and explain physician's instructions.
  • Ability to multi-task with excellent organization skills.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player and ability to work independently.
  • Valid driver's license or Non-Driver photo ID card.

Responsibilities

  • Assess the needs of clients to develop a plan of care.
  • Consult with Management on complex/unique cases.
  • Provide a plan of care with support from multidisciplinary members to ensure services follow the plan of care through assessment, planning, coordination, counseling, teaching, and supervision to meet client/patient needs.
  • Monitor/Assess patients/clients' condition and the effects of the care plan.
  • Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
  • Manage, plan, coordinate, and execute the patient's safe discharge from service.
  • Ensure that physician's orders, discharge summary, and communication to team members, patient, and family members adhere to company and regulatory requirements.
  • Communicate and consult with the patient's MD when needed, manage and coordinate all aspects of the assigned caseload.
  • Assist patients with activities of daily living.
  • Properly document the content and process of all services provided in the internal system.
  • Transport/utilize medical equipment to and from clients/patients' homes/facilities.
  • Travel to patient/clients' homes and/or other facilities.
  • Perform other related duties and participate in special projects as assigned.
