About The Position

Alvita Care is seeking experienced, CT-licensed Registered Nurses (RNs) to provide private duty nursing services. Ideal candidates are reliable, compassionate, and willing to deliver consistent, one-on-one care. This is a per diem position with shifts ranging from 6 to 12 hours, available for both day and night shifts. Opportunities include skilled nursing visits and providing 1:1 care in the home environment. This role offers flexibility to work part-time or around your current schedule.

Requirements

CT RN licensure.
BLS, ACLS, or CPR certification.
Minimum of One (1) year demonstrated experience as an RN.
Ability to put patients at ease and explain physician's instructions.
Ability to multi-task with excellent organization skills.
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
Knowledge of medical terminology.
Team player and ability to work independently.
Valid driver's license or Non-Driver photo ID card.

Responsibilities

Assess the needs of the clients to develop a plan of care.
Consult with Management on complex/unique cases.
Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients.
Monitor/Assess patients/clients' condition, effects of the care plan.
Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
Manage, plan, coordinate and execute the patient's safe discharge from service.
Ensure that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
Communicate and consult with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
Assist patients with activities of daily living.
Document content and process of all services provided in internal system properly.
Transport/utilize medical equipment to and from clients/patients' home/facilities.
Travel to patient/clients' homes and/or other facilities.
Perform other related duties and participate in special projects, as assigned.
