About The Position

James River Home Health is seeking a compassionate and dedicated Home Health RN Case Manager to support patients in the Roanoke and surrounding areas. This role involves managing a caseload of patients in their homes, collaborating with an interdisciplinary team to ensure high-quality care, and joining a rapidly growing, locally owned company committed to exceptional patient care.

Requirements

Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available during work hours.

Nice To Haves

At least one year of home health experience preferred.
OASIS and HCHB experience PLUS!

Responsibilities

Provides skilled nursing care to homebound patients in accordance with the established Plan of Care.
Develops and implements individualized treatment plans while monitoring and reporting changes in condition.
Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges.
Collaborates closely with the interdisciplinary team.
Educates patients and families on health maintenance and disease prevention.
Completes timely electronic documentation in Homecare Homebase.
Offers guidance and emotional support throughout the care journey.
