About The Position

The Registered Nurse is responsible for nursing care and maintaining a safe abuse free environment for all residents in the Orchards of Alliance Skilled. This role involves teamwork, accurate documentation of resident conditions, recognizing significant changes, completing assessments, notifying physicians and families of changes, and overseeing care provided by licensed practical nurses, nursing assistants, and restorative aides. The nurse must immediately report substandard care to the Director of Nursing, conduct daily rounds, verify physician orders, evaluate unit functions for resident care and efficiency, and ensure nursing practice complies with all relevant regulations and policies. Communication with staff, ancillary departments, home care providers, and family members is crucial. The role also includes initiating intervention in abusive situations, assisting with investigations, providing and receiving change of shift reports, administering medications and treatments, ordering medications, maintaining controlled substance counts, participating in in-services and committee meetings, and identifying staff training needs.

Requirements

  • Graduate of an accredited school of nursing and successful passing of registered nursing licensure exam.
  • Complete annual state mandated training requirements (Regular In-services as well as any external training).
  • Familiarity with keyboarding, word processing and software usage.
  • Current Registered Nurse license in West Virginia required.
  • 12 continuing education hours required annually.
  • First Aid and CPR certification.
  • Nursing skills specific to geriatric residents are needed.
  • May need additional skills with special-need geriatric residents such as those with dementia, Alzheimer's or those requiring advanced health care.
  • Ability to use lift equipment.

Nice To Haves

  • IV certification preferred.

Responsibilities

  • Assess, report, and maintain accurate documentation regarding resident conditions.
  • Recognize significant changes in the conditions of residents.
  • Complete assessments as required based on level of care.
  • Notify physician and families/responsible party regarding changes.
  • Oversee care provided on the nursing units by licensed practical nurses, nursing assistants, and restorative aides.
  • Immediately report substandard care to Director of Nursing.
  • Conduct daily rounds on unit and with physician as needed.
  • Complete and verify monthly physician orders.
  • Evaluate functions of unit to ensure resident care, staff relations and efficiency of service.
  • Report any and all concerns to the Director of Nursing.
  • Assure nursing practice is in compliance with Nurse Practice Act, state and federal regulations and facilities policies and procedures.
  • Communicate with staff, ancillary departments, Home Care Providers and family members to discuss issues related to the resident’s condition of care.
  • Initiate immediate intervention in abusive situations to protect the resident and report allegations according to facility policy.
  • Assist with the investigation process when needed.
  • Receive and provide change of shift report regarding resident status and changes in condition.
  • Assist with administration of medications and treatments as prescribed by the physician.
  • Order medications on a routine and as needed basis.
  • Maintain accurate controlled substance count.
  • Participate in Mandatory nursing in-services and committee meetings as required.
  • Identify areas of staff training required to provide quality care and notify Director of Nursing immediately.
