About The Position

As part of The Orchards team, you can help our residents experience the best service and quality care during their nursing home, assisted living, rehab and independent living stays. Under the direction of the nursing staff, cares for residents while keeping them safe and comfortable during their activities of daily living by performing the following duties.

Requirements

  • Certificate upon completion of an approved nurse's aide certification curriculum.
  • Complete annual state mandated training requirements (Regular In-services as well as any external training).
  • Certified Nurse Assistant certification.
  • A Certified Nurse Assistant is required to work 8 hours and take 12 hours of training annually.
  • Certified Nurse Assistant skills specific to geriatric residents are needed.
  • May need additional skills with special-need geriatric residents such as those with dementia, Alzheimer's or those requiring skilled health care.
  • Ability to use resident lift equipment.
  • Regularly required to stand or walk throughout the facility.
  • Regularly required to: complete duties and provide therapeutic touch; reach with hands and arms; and balance.
  • Regularly required to use the telephone or computer, use medical equipment to assess resident vital signs, prepare residents to be lifted, operate the Sit to Stand Maxi/Hoyer lifts, lift or assist a resident out of a chair or bed, feed residents, serve meals, make beds and change linens, dress residents, fold laundry, turn door knobs, reposition chair alarms, assist or perform personal hygiene care for resident (brushing teeth, bathing or showering, drying after bath or shower), manipulate walkers and wheelchairs, assist residents when walking, store clean linens in closets, push residents in wheelchairs, and put away dishes, glasses, etc.
  • Regularly required to stoop, kneel, or crouch during specific tasks.
  • Required to talk or hear on an on-going basis when communicating with various individuals.
  • Frequently required to use his/her senses of taste and smell.
  • Occasionally required to sit at a desk when charting and at a table to feed residents.
  • Frequently lifts and/or moves up to 100 pounds when assisting a resident out of bed or a chair with the aid of lift equipment or assist of two and when transporting residents by wheelchair.
  • Occasionally lifts and/or moves more than 100 pounds when assisting or transporting heavier residents with the aid of lift equipment or the assist of two.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
  • Reading and writing reports
  • Recording of vital signs
  • Watching over residents for their safety and monitoring their behavior
  • Keeping hallways and rooms safe
  • Measuring liquids for input and output recording
  • Reading thermometers
  • Assessing residents' skin texture and color
  • Observing resident’s breathing pattern from doorway at evening rounds
  • Dressing a resident or to identify proper adult protection for incontinence as they are color and size coordinated
  • Using devices to check color-coded wander guard bracelets
  • Assisting residents in walking with walker

  • Additional skills with special-need geriatric residents such as those with dementia, Alzheimer's or those requiring skilled health care.

Responsibilities

  • Teamwork with the following and all other duties and responsibilities assigned.
  • Provides care and services to residents as defined by individualized care plans. Care and services include, but are not limited to: ADL assistance, mobility assistance, positioning assistance and feeding assistance.
  • Performs random rounds on each resident every two hours, starting upon arrival and finishing just prior to the end of the shift.
  • Takes and records temperature, pulse and respiration rates, and food and fluid intake and output, as directed.
  • Participates in resident activity program.
  • Documents care provided to residents using approved documentation method.
  • Reports any change in resident condition to the charge nurse immediately.
  • Assists with stocking supplies in clean utility rooms.
  • Attend meetings and training sessions required for the position
