Registered Nurse - Palliative Home Care
About The Position
Mercy Home Health, a member of Trinity Health at Home, is seeking a dedicated full-time Registered Nurse for its Palliative Care program. The position is based out of South Broad Street and supports North, South, and West Philadelphia. The nurse provides one-to-one care to patients in their homes, with a patient-centered focus and support from colleagues. Requirements
- Bachelor of Science in Nursing (BSN) required.
- Graduate of an approved nursing education program.
- Current Registered Nurse license in Pennsylvania.
- Current CPR certification.
- Hospice/Palliative Care Certification required; the posting also identifies Palliative Care Certification as required.
- Strong interpersonal, organizational, problem-solving, critical-thinking, and priority-setting skills.
- Ability to work independently and as part of a team.
- Proficiency in computer skills.
- Current driver's license and reliable transportation.

- Hospice experience.
- One year of experience as a professional care nurse in home care or hospice care is a plus.
Responsibilities
- Provide comprehensive care to patients and families in the palliative care program.
- Offer clinical consultation in palliative care.
- Develop, coordinate, and evaluate palliative and end-of-life care programs.
