About The Position
Home Health Care, Inc. is seeking a Registered Nurse (RN) Visit Nurse to provide skilled home health visits in the St. Paul, MN area and surrounding communities. This full-time role offers the opportunity to build meaningful, long-term patient relationships and make a lasting difference in patients' lives by helping them regain confidence, manage chronic conditions, prevent hospital readmissions, and remain safely in their homes. Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, acute care, rehabilitation, long-term care, or skilled nursing
- Strong clinical assessment and decision-making skills
- Excellent communication and patient education abilities
- Self-motivated with strong organizational skills
- Reliable transportation for local travel throughout the St. Paul service area
Responsibilities
- Provide skilled RN home visits throughout the St. Paul territory
- Complete comprehensive patient assessments
- Perform wound care, medication management, injections, and other skilled nursing procedures
- Develop and update individualized care plans
- Educate patients and caregivers on medications and disease management
- Communicate with physicians and interdisciplinary care teams
- Complete timely documentation using Homecare Homebase
- Monitor patient progress and coordinate ongoing care
