About The Position

Health Link Home Health and Hospice is seeking compassionate, organized, and team-oriented Registered Nurses (RNs) for multiple territories in the East Bay. The company has been a trusted provider of home health services in the San Francisco Bay Area for nearly two decades. RNs will collaborate with experienced clinicians and receive support from a dedicated back-office team. This is a full-time position with multiple territories available.

Requirements

• Current California RN License (Required)
• Current CA Driver’s License
• Excellent organizational and communication skills
• Ability to work independently in the field

Nice To Haves

• Home Health Experience: 1 year (Preferred)

Responsibilities

• Provide skilled nursing services within the RN scope of practice and in accordance with the individualized plan of care
• Follow the established plan of care, deliver treatments, evaluate patient response, and promptly report any changes in condition to the Physician, Case Manager, and/or Director of Nursing
• Maintain accurate, timely clinical records and documentation in accordance with organizational policy using the company-issued device
• Provide supervision to Licensed Vocational Nurses (LVNs) when assigned, ensuring adherence to the plan of care and quality standards
• Collaborate with the Interdisciplinary Team to support coordinated, patient-centered care
• Follow all company policies and procedures
• Perform other duties as assigned
