About The Position

Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients and their families, and the referring agency. This role involves patient contact with individuals of all ages in a clinical setting, requiring appropriate interaction skills across all populations. Requirements

  • One (1) year experience as an RN in a clinical care setting.
  • Nursing skills as defined as generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision making skills.
  • If required to make visits, Hepatitis profile.
  • Must read, write and comprehend English.
  • Completion of an accredited Registered Nursing Program.
  • Current driver’s license in good standing.
  • Reliable transportation and auto liability insurance.
  • Current State professional license as an RN in any/all applicable states.
  • Current CPR.
  • On-going employment as an RN requires maintenance of a valid RN license in the state which the nurse practices in and a valid CPR certificate at all times.

Responsibilities

  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health clients, including OASIS assessments.
  • Regularly reevaluate client nursing needs and evaluate care outcomes, assisting clients in recovery or maintenance of life functions and preventing illness/relapse.
  • Develop, initiate, and revise the Plan of Care (POC) as necessary, initiating appropriate preventative and rehabilitative nursing procedures, and planning for discharge.
  • Provide effective communication to clients, families, team members, and other healthcare professionals, with regular participation in Case Conferences.
  • Apply infection control and universal precaution concepts in client care activities to protect clients and staff.
  • Coordinate services and inform the physician and other personnel of changes in client condition and needs.
  • Monitor assigned cases for compliance with third-party payer requirements.
  • Complete appropriate documentation in a timely manner.
  • Supervise, teach, and provide clinical direction to other nursing personnel (e.g., LVN/LPN, HHA, CNA).
  • Perform on-call responsibilities and provide on-call services to clients and families.
  • Demonstrate commitment and professional growth through in-service programs and maintaining/improving competency.
  • Meet mandatory continuing education requirements.
