About The Position

DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, manages a PD patient caseload, monitors treatment progress, conducts home visits, and provides on-call support. The nurse collaborates with interdisciplinary teams to help ensure safe, high-quality outcomes.

The schedule is Monday through Friday in 8-hour shifts, with weekends and holidays off. Scheduling is flexible to support management of the nurse’s caseload. The position is based at 23 Warren Ave, Woburn, Massachusetts.

Requirements

• Registered Nurse license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate Degree in Nursing (ADN) required.
• Basic computer skills, including MS Word and Outlook.
• At least 1 year of RN experience.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Dialysis experience, including PD, HHD, chronic, or acute dialysis.
• Patient education or teaching background.
• Case management or telehealth experience.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on peritoneal dialysis procedures and self-care.
• Manage a caseload of peritoneal dialysis patients, including assessments and care planning.
• Monitor treatment progress.
• Conduct home visits.
• Provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
