About The Position

Pinnacle Home Care is seeking Registered Nurses to provide compassionate, patient-centered home health care. The role focuses on individualized care planning and nursing services that support patients’ recovery, health, and independence in their homes. Nurses conduct health and psychosocial assessments, monitor patient conditions, coordinate care and health promotion, and maintain accurate clinical records, including required OASIS documentation. They also educate and support patients and caregivers, coordinate with clinical colleagues, physicians, discharge planners, and community health resources, and follow agency policies and quality processes. The role involves visiting patients’ homes, including homes that may not be wheelchair accessible.

Requirements

• Current state-specific Registered Nurse license.
• Valid BLS certification.
• At least one year of experience in home health or a healthcare facility.
• Familiarity with Medicare and Medicaid guidelines.
• Solid understanding of professional nursing principles and home health documentation.
• Excellent communication, organizational, and time management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Responsibilities

• Design and implement patient-specific care plans focused on optimal recovery and improved health.
• Provide nursing care in accordance with each patient’s care plan, including comprehensive health and psychosocial assessments.
• Monitor patients’ conditions and coordinate health promotion, prevention, and direct care.
• Maintain detailed and accurate clinical records, including required OASIS documentation.
• Communicate with clinical managers and colleagues to support high-quality care.
• Train and support patients and their families or caregivers.
• Serve as a point of contact between patients and community health resources.
• Work with physicians and discharge planners to provide health education to patients and caregivers.
• Confirm daily visit schedules to coordinate with other staff.
• Demonstrate awareness of and participate as required in the agency’s Quality Assurance and Performance Improvement (QAPI) program.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with agency service agreements and admission policies.
• Follow agency policies regarding appropriate patient discharge and reasons for discharge.
