About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients recover and regain independence in their homes. You work with physicians and clinicians to manage care plans that support patients’ recovery. The position involves providing skilled nursing care in patients’ homes, developing and implementing individualized care plans, monitoring patient conditions, educating patients and families, maintaining care records, coordinating with healthcare professionals, reporting patient progress, and overseeing Home Health Aides. This is a patient-facing role subject to Humana’s TB screening program and driver safety requirements. Scheduled weekly hours are 40. The posting lists a pay-per-visit/unit rate of $49.00–$69.00 and a full-time annual base pay range of $77,200–$106,200; pay varies based on location and job-related qualifications.
Requirements
- Med-surg, ICU, ER, or acute-care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families about disease management, medication, and treatment options.
- Maintain accurate patient-care records and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor appropriate completion of their documentation.
