About The Position

VitalCaring is hiring a Home Health Registered Nurse to provide field-based care to patients in their homes within an assigned territory. The RN serves as the clinical lead, managing each patient’s plan of care, coordinating across disciplines, and driving outcomes from admission through discharge. The role combines advanced assessment, care planning, skilled nursing, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home. The position offers clinical autonomy and ownership, care for patients with complex needs, meaningful ongoing relationships with patients and families, and support from a collaborative clinical team. The work requires adaptability, critical thinking, time management, and performance against expectations for clinical quality, patient outcomes, and timely documentation.

Requirements

Active RN license or compact eligibility.
CPR certification.
Graduate of an accredited nursing program.
Minimum one year of RN experience.
Strong clinical judgment and ability to think independently.
Excellent communication skills and a patient-centered approach.
Ability to manage a caseload and prioritize effectively.
Proficiency with EMR systems.
Valid driver’s license and reliable transportation.

Nice To Haves

Home Health, Med-Surg, ICU, or Acute Care RN experience is preferred.
Experience in home health or post-acute care.
Experience with case management and interdisciplinary coordination.
BSN.
Familiarity with Homecare Homebase (HCHB).

Responsibilities

Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
Develop and manage individualized care plans in collaboration with physicians and care teams.
Deliver skilled nursing care, including medication management, treatments, and interventions.
Serve as case manager, coordinating care across therapists, aides, and physicians.
Monitor patient progress and make clinical decisions based on changing conditions.
Educate patients and families to support disease management and independence.
Coordinate care transitions and discharge planning to reduce readmissions.
Maintain timely, accurate documentation in the EMR.
Supervise and support Home Health Aides as needed.
Participate in an on-call rotation as appropriate.
