About The Position

The Registered Nurse (RN) at the Galveston County Mental Health Wellness Center is responsible for providing comprehensive nursing care to individuals with mental illness, co-occurring substance use/abuse diagnoses, or dual diagnoses of mental illness and intellectual/developmental disabilities. This role supports three 24-hour observation beds, nine extended observation beds, and twelve intensive crisis respite beds. The RN functions as a key member of the interdisciplinary team, utilizing the nursing process to plan, implement, document, and evaluate patient care in accordance with the Texas Nurse Practice Act. Key responsibilities include ensuring medical admission criteria are met, collaborating with state and local hospitals, and maintaining a safe environment for patients and staff. The position operates under a trauma-informed approach, emphasizing safety, trustworthiness, peer support, collaboration, empowerment, and cultural sensitivity.

Requirements

Associate degree Nurse (ADN) OR Diploma Nurse (RN) from a three-year college OR Bachelor's degree in nursing (BSN) from a four-year college or university.
2 years registered nursing experience (not including clinical hours required for education).
Registered Nurse licensed to work in Texas.
General knowledge of mental health disorders.
Ability to establish cooperative relationships with youth, their families, and their communities.
Ability to provide sensitivity toward family needs and preferences.
Ability to work cooperatively with immediate work group and individuals external to the program.
Knowledge of local social service network.
Self-motivated with good time management skills.
Mature problem-solving approach to daily work.
Ability to express self clearly and effectively, orally and in writing.
Demonstrated PC skills.
Ability to work within a team approach.
Proficient in the use of electronic data systems including electronic health records and business software such as Microsoft Word and Excel.
Ability to transport, adjust, and/or move items up to 55 lbs.
Ability to ascend/descend stairs.
Ability to remain in a stationary position for long periods of time.
Ability to handle objects.
Ability to operate office equipment.
Ability to move around within an office setting and/or other locations as needed; may include ability to bend/twist, kneel/stoop and/or reach for items.
Ability to push/pull, crawl/squat as it relates to PMAB requirements; may be required to perform PMAB restraints as it relates to program requirements.
Ability to detect and write effectively.

Ability to have visual acuity to operate a motor vehicle and observe objects within work setting.

Responsibilities

Provide nursing care to individuals with mental illness, co-occurring substance use/abuse diagnoses, and dual diagnoses.
Coordinate nursing activities as a member of the interdisciplinary team.
Utilize the nursing process to plan, implement, document, and evaluate nursing care.
Ensure medical admission criteria are met for all patients.
Work closely with state and local hospitals and other staff to ensure patient and staff safety.
Perform tasks as assigned by supervisors.
Float to various units within the Galveston County Mental Health Wellness Center, including observation and crisis respite beds.
Review admission information and follow up with referring providers.
Provide documentation of pending admissions to the oncoming shift.
Facilitate the timely intake of clients.
Plan and implement nursing care that reflects sound clinical decision-making.
Participate in team rounds as a client advocate.
Perform clinical nursing skills within the scope of practice for a Registered Nurse.
Report medication errors immediately.
Provide nursing care with a focus on patient, environmental, and technical safety.
Assign specific tasks and maintain supervision of LVNs and unlicensed personnel.
Coordinate and document referrals to the clinic, integrated health, and ER.
Provide discharge referrals to appropriate medical services.
Perform thorough assessments of patients by collecting and analyzing data.
Perform diagnostic tests, administer medications, set up equipment, and apply therapeutic treatments.
Administer medications according to the five rights of administration.
Perform medication and diagnosis education.
Inform, explain, and educate patients and family members regarding medications, treatment information, and disease processes.
Provide group education on health-related topics.
Document relevant information regarding nursing care in patient records, charts, and reports.
Document observations of patient signs, symptoms, test results, behaviors, and response to medications and treatments.
Document daily Columbus Suicide Rating for all EOU and Crisis Respite clients.
Comply with established agency policies/procedures, accrediting/regulatory bodies, and performance contract documentation.
Transcribe and implement physician orders accurately.
Implement prescribed actions for 'panic' values (lab or diagnostic results).
Utilize correct nursing codes to accurately document nursing services.
Document behavioral incidents and injuries within the electronic health record.
Communicate regularly with supervisor.
Consult and coordinate with an interdisciplinary team.
Utilize supervisor and on-call support for problem-solving and incident reporting.
