The position focuses on assessing patient needs, planning care, coordinating assignments, participating in team meetings, establishing professional relationships, evaluating and documenting outcomes, communicating with team members, serving as a resource for complex situations, and participating in education programs. The role also involves administering medication accurately and providing direct patient care, including administering medications and documenting care based on scientific principles and the nursing process. Education is a key component, involving planning and providing education to patients, families, and caregivers, and developing educational materials. Managing the patient's hospital stay based on anticipated length of stay, clinical path, and protocols is also crucial, requiring collaboration with various healthcare professionals for discharge planning and timely completion of procedures. Discharge planning begins on day one for all patients, with family involvement, and all patients returning home receive a written discharge or instruction plan.
