About The Position
Community Home Healthcare is growing and seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join their team. Requirements

Responsibilities
- Provide skilled nursing visits (RN's/LPNs) based on the client’s plan of care.
- Medication management, vitals, treatments, wound care, education, and assessments.
- Collaborate with our clinical team for changes in condition and care coordination.
- Document visits electronically (user-friendly system; training provided).
