About The Position

The Registered Nurse (RN) in the Assisted Living Facility (ALF) is responsible for directing nursing care to residents and overseeing daily nursing activities performed by certified nursing assistants and medication aides. This role operates in accordance with federal, state, and local regulations, as well as facility policies and procedures, promoting holistic care for residents.

Requirements

• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Registered Nurse (RN) in practicing state.
• Current CPR certification.
• Ability to read, write, speak, and understand the English language.
• Must be a supportive team member, contribute to and be an example of teamwork.
• Ability to make independent decisions when circumstances warrant such action.
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must have patience, tact, and willingness to deal with difficult residents, family, and staff.
• Must be able to relay information concerning a resident’s condition.
• Must not pose a threat to the health and safety of other individuals in the workplace.
• Must be able to move intermittently throughout the workday.
• Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status for various infectious diseases.
• Ability to assist in evacuation of residents during emergency situations.
• Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
• Must possess leadership, supervisory ability, and willingness to work harmoniously with and supervise other personnel.
• Must be able to follow oral and written instructions.
• Communicates with medical and nursing staff, and other departments.
• Agreement to abide by all standards, policies, and procedures of the facility, including the facility’s compliance and ethics program, is a condition of employment.

Responsibilities

• Directs the daily activities of certified nursing assistants and caregivers.
• Follows the nursing model to promote holistic care for residents.
• Ensures compliance with policies and procedures by nursing personnel.
• Participates in resident admissions, discharges, and transfers.
• Assesses changes in resident status, notifying physicians, families, and documenting accordingly.
• Confirms physician orders and carries them out.
• Prepares and administers medications and observes for adverse effects.
• Schedules follow-up appointments and transportation needs for residents.
• Provides nursing leadership to nursing personnel.
• Coordinates care skills such as colostomy changes and wound care.
• Performs rounds to ensure resident needs are met and personnel are performing duties.
• Collaborates with the interdisciplinary team to ensure residents’ needs are met.
• Participates in management team meetings to discuss census, resident status, or concerns.
• May assist in evaluating work performance of nursing personnel and implementing discipline.
• Informs appropriate departments of concerns or complaints and files grievances.
• Reports incidents or unusual occurrences and participates in investigations.
• Participates in end-of-life care.
• Ensures adequate stock of medications, supplies, and equipment.
• Initiates, reviews, and updates care plans.
• Promotes teamwork, mutual respect, and effective communication.
• Participates in survey processes.
• Participates in QAPI activities.
• Ensures infection control principles are followed and corrects deficiencies.
• Provides Medication Technician Training and delegation.
• Ensures a Change of Condition Log is maintained daily.
• Participates and executes proper medication destruction procedures.
• Ensures residents transition timely to home health, hospice, or palliative services.
• Treats all residents with dignity and respect.
• Promotes and protects all residents’ rights.
• Establishes a culture of compliance by adhering to all facility policies and procedures.
• Complies with standards of business conduct, and state/federal regulations and guidelines.
• Follows appropriate safety and hygiene measures.
• Maintains confidentiality of protected health information.
• Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality.
• Reports any retaliation or discrimination.
• Reports any allegations of abuse, neglect, or mistreatment of residents.
• Protects residents from abuse and cooperates with investigations.
• Reports occupational exposures to blood, body fluids, or hazardous chemicals.
• Participates in life safety and emergency drills and training.
• Fulfills responsibilities during emergency plan implementation or activation.
• Reports work-related injuries and illnesses immediately.
• Follows established infection control policies and procedures.
• Completes all assigned training and skills competency.
