About The Position

Baylor Scott & White Continuing Care Hospital (CCH) offers a unique blend of ICU-level care and Med-Surg complexity within one specialized hospital unit. Our teams provide high-acuity, long-term care while building meaningful relationships with patients who typically stay with us longer, allowing for deeper continuity of care. CCH is truly like a small family. Our interdisciplinary teams know each other well, support one another, and share a common goal: doing what’s best for our patients and for each other. We work hard, support growth, and enjoy what we do—together. Our LTAC unit cares for patients with complex medical needs, including ventilator weaning, pulmonary and cardiopulmonary diseases, complex wound care, neuromuscular disorders, gastrointestinal diseases, postoperative complications, kidney disease (including dialysis), and post-trauma recovery. The average patient length of stay is 25 days, and the unit size is 25 beds.

Requirements

Active RN license (Texas or compact state)
Passion for caring for medically complex patients
Desire to grow clinically in a collaborative, close-knit environment
Graduated from an Accredited Program in nursing
BLS certification or obtain within 30 days of starting
A deep understanding of nursing and patient care standards, as well as procedures.
Knowledge of nursing laws, rules, standards, and guidelines in your area.
Understanding of hospital procedures and rules.
Proficiency in medical terms, focusing on health, disease prevention, management, common meds, and their side effects.
Respect for medical and professional nursing ethics and patient privacy rights.
Exceptional communication skills, allowing for clear expression of thoughts both verbally and in writing.
Positive social skills to facilitate interactions with a diverse range of parties.
Excellent problem-solving capabilities, driving effective critical thinking.
Basic computer skills, encompassing areas such as Microsoft Office, information security, scheduling and payroll systems, electronic medical documentation, and email.

ICU, LTAC, or acute care experience preferred

Responsibilities

Oversee patient care using a professional practice model.
Conduct patient assessment, recognizing health issues.
Create care plans and assess patient responses.

Delegate tasks carefully and follow up promptly.
Use different strategies for problem-solving with patients, families, and staff.
Recognize limitations and prioritize safety, effectiveness, and efficiency in patient care planning and delivery.
