About The Position

Home Health Care 2000 is hiring a Registered Nurse to provide case management of home health care services in Baton Rouge, Louisiana. The nurse coordinates and manages patient care based on a physician’s plan of care, oversees home health care personnel, handles service authorizations, and documents patient information using an electronic medical record system. The position requires timely documentation in accordance with medical service plan participation rules. The schedule is an 8-hour shift, Monday through Friday, with on-call responsibilities.

Requirements

At least one year of medical-surgical adult health nursing experience.
At least one year of home health experience (listed as required in the schedule section).
Experience managing care plans.
Experience coordinating multidisciplinary activities and managing home health care personnel.
Self-starter with excellent organizational and writing skills.
Current and active Registered Nurse (RN) license in the State of Louisiana.
Experience with electronic documentation.

Nice To Haves

Experience with current Medicaid and Medicare regulations and Joint Commission home health standards is helpful.
Home health experience is described as preferred in the qualifications section, though the schedule section lists one year as required.
