About The Position

Evergreen is seeking a compassionate and dedicated Registered Nurse to join its 2nd shift team in a short-term rehabilitation setting. The nurse provides high-quality, resident-centered care to promote residents’ health, comfort, and well-being, working independently and collaboratively with an interdisciplinary care team. The position is full-time, scheduled for 30–40 hours per week and 8–10 shifts per two-week pay period, with PM shift hours of 2:30 p.m. to 10:45 p.m.

Requirements

Active Wisconsin Registered Nurse (RN) license.
Strong communication and interpersonal skills.

Nice To Haves

Previous experience in long-term care, rehabilitation, skilled nursing, or a similar healthcare setting.
Experience caring for older adults and individuals with complex medical needs.

Responsibilities

Provide compassionate, high-quality nursing care in a short-term rehabilitation environment.
Assess resident needs and develop individualized care plans.
Administer medications and treatments according to physician orders.
Monitor resident conditions and evaluate care outcomes.
Collaborate with physicians, therapists, nursing staff, and other interdisciplinary care team members.
Build trusting relationships with residents and families through education, communication, and emotional support.
Maintain accurate and timely clinical documentation.
Participate in ongoing education, training, and professional development opportunities.
