About The Position

This role is for a Registered Nurse in the Women and Infant department at All Saints Hospital in Racine, WI. The position is full-time and involves a night shift schedule. The nurse will deliver compassionate, high-quality direct patient care, manage the patient's comprehensive care journey by executing physician and nursing orders, and coordinate patient services across all clinical disciplines. Responsibilities also include documenting and assessing the patient care journey, communicating status updates to the multidisciplinary team, and facilitating comprehensive discharge planning.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Cross accepted.
Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Responsibilities

Deliver compassionate, high-quality direct patient care that strictly adheres to established, patient-first best practices and protocols.
Manage the patient's comprehensive care journey, meticulously executing physician and nursing orders promptly to ensure the timely advancement of treatment plans.
Coordinate patient services across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated care for optimal outcomes.
Document and continually assess the patient care journey, meticulously recording progress and communicating critical status updates to the multidisciplinary team to ensure continuity of care and patient safety.
Facilitate comprehensive discharge planning by proactively assessing patient needs and collaborating with the healthcare team to ensure a safe, smooth, and successful transition home.
