About The Position

Catholic Health Home Care is hiring a Registered Nurse to provide home health care to patients in assigned communities across Long Island. The role involves managing a patient caseload and delivering skilled nursing care, assessments, treatments, and education in patients’ homes. Nurses work with physicians and a multidisciplinary care team. The schedule is five 8-hour shifts, 8:30 a.m. to 4:30 p.m., with rotating weekends. The listed salary range is $100,000–$130,000 per year, with additional compensation for visits over the assigned caseload.

Requirements

• New York State RN license.
• At least one year of recent hands-on experience as a Registered Nurse.
• Basic Life Support certification.
• Valid New York State driver’s license and reliable transportation.

Responsibilities

• Case manage patients and assess, plan, develop, and implement comprehensive home health care plans.
• Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
• Provide skilled nursing care, preventive and rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home situations.
• Work with physicians, therapy and social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
• Educate patients and families about disease management and health promotion as directed by the physician’s plan of treatment.
• Provide care to the highest standard.
