About The Position
The Registered Nurse is responsible for providing nursing care and supervision of care in accordance with the clients’ plan of care.
Requirements
- Must be a graduate of an accredited college or university
- Must be licensed in the State as a Registered Nurse
- Must have a minimum of one year of experience in home health agency or in a hospital setting
- Must be familiar with CAHC Accreditation Standards
Responsibilities
- Perform In-Home Consultation at client's home
- Conducts initial and ongoing assessments
- Create initial and maintain ongoing Plan of Care of the client’s needs
- Conduct 30-day calls and 60-day client visits
- Perform initial and annual HHA case orientation, clinical supervisions, and competency review
- Must be able to communicate promptly with DON and staff
