About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services to help people receive care at home. In this role, the Nurse Case Manager guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The schedule is seven days on followed by seven days off. The role has a combination of per-visit-point pay for visits and hourly pay for certain non-visit activities. Candidates must pass a drug test before beginning employment.

Requirements

Current RN license specific to the state(s) of assignment.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse.
Valid driver's license, reliable transportation, and liability insurance.
Pass a drug test before beginning employment.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Refer patients to other disciplines based on identified needs, or document the rationale for not making a referral.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
