About The Position

The Center Nurse is a clinical care team member at Oak Street Health who works within a Value-Based Care model focused on patient outcomes and long-term wellness. The nurse uses standards, guidelines, and care pathways to support patients and prevent complications. The role delivers clinical assessments and chronic condition management, collaborates with the care team to reduce avoidable hospitalizations, and promotes an engaging, welcoming environment through communication and delegation. Center Nurses report to the Practice Manager.

The position also supports nursing practice and standard operating procedures, identifies barriers to optimal care, contributes to daily care team meetings, supports clinical colleagues and providers, and ensures compliance with patient safety initiatives. The nurse provides emergency care according to organizational procedures and maintains professional competence through ongoing learning, self-evaluation, and evidence-based practice.

Requirements

• Active, unrestricted Registered Nurse licensure in the state where the candidate resides.
• BLS or CPR certification.
• Fluent written and spoken English language skills.
• U.S. work authorization.
• Experience with electronic medical records (EMR).
• Ability to maintain patient confidentiality and handle information confidentially.
• Ability to assess patients without face-to-face interaction, with strong communication and assessment skills.

• Excellent computer skills, including the ability to read, interpret, and analyze data from various computer systems.
• Effective problem-solving and prioritization skills.
• Two or more years of healthcare experience working as an RN.

• Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities served.

Responsibilities

• Support patient assessment and prioritize walk-in and telephone triage to direct patients to the appropriate level of care and help avoid unnecessary hospital visits.
• Provide protocol-driven chronic condition management.
• Perform skilled nursing procedures, including wound care and medication administration, including vaccines.
• Educate patients and caregivers about chronic conditions and preventative care in person, by phone, or in group settings.
• Conduct medication reviews during nurse-led visits to support patient safety and adherence.
• Perform onsite testing, including point-of-care labs, EKGs, and specimen collection, and use patient data to identify and close gaps in preventative screenings in compliance with state scope of practice.
• Review and act on high-priority abnormal laboratory results in a timely manner.
• Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients returning to the clinic from the hospital.
• Use standardized protocols for medication management, prescription renewals, and prior authorizations.
• Manage clinical communications, complete tasks, and close patient visit encounters in a timely manner.
• Delegate to medical assistants and phlebotomists as needed to support patient flow and efficiency.
• Coordinate, integrate, communicate, and implement nursing practice and standard operating procedures.
• Identify barriers to optimal care, including physical, cognitive, language, literacy, socioeconomic, cultural, and behavioral barriers.
• Create a welcoming and engaging environment for patients, communities, families, and teams.
• Deliver a positive patient experience through responsive, respectful care.
• Contribute as a clinical voice in daily care team meetings to align on patient priorities and clinic operations.
• Serve as a support and resource for nursing staff, other clinical staff, and providers.
• Ensure clinical compliance with patient safety initiatives and reporting.
• Provide emergency care in accordance with organizational procedures.
• Engage in self-evaluation and professional growth and development activities.
• Maintain current nursing knowledge and competence, apply patient-centered evidence-based practice, and consider relevant research in care delivery.
• Identify and appropriately report professional misconduct.
• Perform other related duties as assigned.
