About The Position

Catholic Health Home Care is hiring a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes case management and developing and implementing comprehensive home health care plans. Nurses work with a supportive patient care team and provide patient and family education as directed by the physician’s plan of treatment. The position uses 8-hour shifts from 8:30 AM to 4:30 PM, with rotating weekends. Per-visit nurses must attend a full week of orientation Monday through Friday, from 8:30 AM to 4:30 PM, and work one shift per pay period. The stated pay range is $62.00–$135.00 per hour; actual pay depends on qualifications, skills, competencies, and experience.

Requirements

New York State Registered Nurse license.
At least one year of recent hands-on experience as a Registered Nurse.
Basic Life Support certification.
Valid New York State driver’s license and reliable transportation.

Responsibilities

Case manage patients and assess, plan, develop, and implement comprehensive home health care plans.
Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
Provide skilled nursing care, preventive and rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home situations.
Collaborate with physicians, therapy and social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
Provide patient and family education for disease management and health promotion as directed by the physician’s plan of treatment.
Provide care to the highest standard.
