RN On-Call - 7 on, 7 off
About The Position
Continuum Hospice is currently seeking a Full Time On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments. As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support. Schedule: 7 Day Rotation, Monday - Friday, 4:30 pm - 8 am Territory: Contra Costa & Alameda County At Continuum Hospice, we take immense pride in being a mission-driven, patient-centered leader in end-of-life care. Requirements
- Hold current unencumbered license as a Registered Nurse.
- Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR One (1) year experience as a professional nurse within the last three (3) years.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Nice To Haves
- Hospice/home health experience preferred but not required.
Responsibilities
- Respond to a variety of patient needs, from urgent to routine, your unwavering support will make a difference in critical moments.
- Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
- Maintain accurate documentation, both in the patient's home and within our agency and regulatory standards.
- Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones.
