About The Position
ComForCare is hiring an experienced Registered Nurse in Greeley, Colorado, and neighboring cities. The role lets the RN manage a client caseload, guide caregivers, and build relationships with clients and families in a home care setting. The position is full-time, pays $47–$52 per hour based on experience, and follows Monday–Friday day shifts from 8 a.m. to 5 p.m., with flexible scheduling, occasional holidays, and a weekend rotation. The RN completes in-home client assessments, develops and updates individualized care plans, oversees clients’ changing needs, supervises and trains caregivers, coordinates care with families and health care partners, maintains compliant documentation, and provides on-call support. ComForCare describes the role as offering independence in home care with organizational support.
Requirements
- Active, unencumbered Registered Nurse (RN) license in Colorado.
- At least 1 year of home care or home health nursing experience.
- Current BLS/CPR certification.
- Valid driver’s license and reliable transportation.

- Strong clinical judgment, organization, and communication skills.
- Genuine passion for client-centered care.

- Case management, care coordination, or caregiver supervision experience.
Responsibilities
- Complete initial and ongoing in-home nursing assessments of clients’ physical, cognitive, psychosocial, and home safety needs.
- Create and update individualized plans of care and ensure caregivers understand and follow them.
- Manage a caseload of home care clients, track changes in condition, and adjust services as needs change.
- Conduct supervisory visits and complete caregiver competency checks.
- Serve as the main clinical contact for families, physicians, hospitals, and community partners; make referrals when needed.
- Maintain accurate, timely client records and follow Colorado home care regulations, infection control, and HIPAA guidelines.
- Provide on-call support to resolve client and caregiver concerns quickly and professionally.
