Requirements
- Active RN license in good standing (Virginia state).
- Graduate of an accredited nursing program (ADN or BSN).
- Active CPR / BLS certification.
- Valid driver’s license, reliable vehicle, and active auto insurance.
- Strong clinical judgment, exceptional communication, and a patient-first heart.
Nice To Haves
- 1+ years of home health or home care experience preferred.
Responsibilities
- Perform health evaluations and deliver individualized care in patient homes.
- Evaluate physical, psycho-social, and home environment factors to tailor care plans.
- Teach patients and family caregivers how to manage health conditions effectively.
- Work alongside multidisciplinary teams to ensure complete, continuous care.
- Complete timely visit reports and provide supportive guidance to home care aides.
