Requirements
- Active Registered Nurse (RN) license in good standing (Virginia state).
- Graduate of an accredited nursing program (Associate, or Bachelor’s Degree).
- Current CPR / BLS certification.
- Valid driver’s license, reliable transportation, and active auto insurance.
- Strong clinical judgment, excellent communication skills, and a compassionate, patient-first mindset.
Nice To Haves
- 1 year of RN home health/home care experience preferred.
Responsibilities
- Conduct regular, personalized visits to evaluate and support patients' needs.
- Thoroughly assess physical, psycho-social, and environmental factors in the home.
- Empower families with clear guidance and resources to enhance quality of life.
- Partner with multidisciplinary healthcare teams to ensure seamless care delivery.
- Maintain timely and accurate visit documentation, and provide guidance to home care aides.
