About The Position

Uplift HomeCare, LLC is seeking a compassionate and dependable Registered Nurse to provide quality in-home care to patients in Gibsonton, Florida, and surrounding areas. The role focuses on helping seniors and Veterans maintain their independence and receive care in the comfort of their homes. The position is PRN or part-time with flexible scheduling.

Requirements

Current, unrestricted Florida Registered Nurse (RN) license.
Current CPR certification.
AHCA Level II background screening, as required.
Valid driver’s license, reliable transportation, and current auto insurance.
Strong clinical judgment, communication skills, organization, and attention to detail.

Nice To Haves

Home health experience is preferred; experience requirements may vary by assignment.
OASIS experience is strongly preferred.
Experience with Axxess or another home health EMR is a plus.

Responsibilities

Perform skilled nursing assessments and evaluate patient health status.
Develop, implement, and update individualized plans of care.
Provide nursing treatments and monitor patients for changes in condition.
Administer medications and treatments as ordered and within the RN’s scope of practice.
Educate patients and caregivers on diagnoses, medications, safety, and disease management.
Coordinate care with physicians, therapists, and other members of the healthcare team.
Complete accurate, timely clinical documentation, including OASIS assessments when applicable.
Follow applicable state and federal regulations, infection-control practices, HIPAA requirements, and agency policies.
