About The Position

Amedisys is hiring a registered nurse to provide hospice care and on-call services to patients and families. The on-call schedule is Friday, Saturday, and Sunday from 5 p.m. to 8 a.m. The role includes assessing new admissions, coordinating and maintaining patient care plans, communicating progress with the clinical manager, physician, and care team, documenting care, educating families, and making referrals to other hospice disciplines as needed. The position offers pay of $85,000 to $90,000. Benefit eligibility may vary by position and shift status; participation in certain benefits requires enrollment in an Amedisys medical plan.

Requirements

• Current RN license specific to the state or states assigned.
• At least one year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform initial nursing assessments on new admissions.
• Complete, coordinate, and revise the total plan of care, and maintain continuity of patient care in collaboration with appropriate staff.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Develop, prepare, and maintain accurate and timely individualized patient care progress records according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Educate families about the patient's condition, needs, and care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Perform assigned on-call responsibilities and services for patients and families.
• Perform other duties as assigned.
