Requirements

  • Recent experience within last two years in acute and/or sub-acute/long term care.
  • Registered Nurse: CA, upon hire
  • Basic Life Support - CPR, within 30 Days

  • Utilizes the nursing process to provide safe therapeutic care for patients

  • Utilizes the nursing process to participate in discharge planning activities.
  • Identifies need for social services interaction regarding family issues and communicates the issue appropriately.
  • Completes the nursing portion of the MDS in a timely manner to ensure compliance with Medicare guidelines.
  • Provides patient care for a given patient assignment, either as a primary caregiver or as a member of a care team.
  • Works collaboratively with physician by making rounds, reviewing orders with physician, and reporting abnormal lab values and significant changes in patient findings in a timely manner.
  • Works collaboratively with patient and family and the healthcare team to obtain desired patient outcomes in a timely manner.
  • Evaluates family strategies in relation to patient.
  • Provides patient and family teaching necessary to meet the needs of the patient upon discharge.
  • Identifies and intervenes appropriately in crisis situations.
  • Demonstrates technical nursing skills (as documented in unit skills list).
  • Documentation of the nursing process reflects basic level of practice as defined in general description.
  • Assigns patients based on their nursing care needs and the competencies of staff members with consideration of unit financial budget.
  • Functions as the charge nurse to supervise and direct patient care on the unit for an assigned shift.
  • Functions as a resource person to co-workers in the assessment of patient on the unit.
  • Demonstrates the ability to organize and prioritize efficiently.
  • Participates in continuing education in areas applicable to patient population.
