About The Position

Joseph & Mary’s Home and the Sisters of Charity Health System are committed to providing respectful, compassionate respite care for adults experiencing homelessness and acute medical issues in an environment that is Trauma Informed, Culturally Competent, and medically appropriate. The RN helps residents stabilize their health, connect to housing and needed supports and rebuild their lives. Under the supervision of the Medical Director, and reporting to the Executive Director the RN provides holistic nursing support, resident care and medication management.

Requirements

  • Must have an understanding of the factors related to housing instability and the relationship between stable housing and other social factors (i.e., the social determinants of health) as they relate to positive health outcomes.
  • Graduate of an accredited school of nursing.
  • Licensed in Ohio as a Registered Nurse.
  • Prior experience and working with electronic medical record and database systems (InSync).
  • At least five years of experience working in acute care or nursing home settings.
  • Understands HIPAA requirements and maintains client confidentiality at all times.
  • Commitment to leading and working collaboratively as part of a team.
  • Is a highly organized, detail-oriented person with excellent communication skills.
  • Demonstrates high standards of accountability and professional conduct.
  • Possesses a valid Driver’s License and current BLS certification.
  • Knowledge and experience working with people who have medically and socially complex issues, particularly SMI and/or SUD issues.

Nice To Haves

  • Familiarity with CARF and OhioMHAS standards strongly preferred.

Responsibilities

  • Conducts a baseline assessment of each resident within 72 hours of admission to determine factors that will influence care, treatment and services.
  • Perform primary nursing procedures to include wound care, glucometer testing, insulin administration and vital signs interpretation.
  • Administer medications as ordered by providers.
  • Integrate any new orders into resident charts and resident integrated care plan.
  • Monitor and document any change in health status, including but not limited to ability to perform ADLs, IADLs, changes in mood, changes in mental status, etc. and notify the Medical Director of the changes.
  • Assist the Medical Director with determining and documenting medical stability as a part of resident discharge.
  • Teach and encourage residents’ self-care practices regarding care of their illnesses, management of their medications and activities of daily living.
  • Perform reconciliation of medication for all residents on a weekly basis.
  • Ensure accuracy of eMAR.
  • Perform quality control checks of glucometers and other equipment.
  • Ensure supplies in medical clinics are stocked and organized.
  • Communicate with pharmacies to obtain medications ordered by the provider; assist residents with consolidating prescriptions to a single community pharmacy and establishing an ongoing relationship.
  • Assist residents with navigating health systems and establishing ongoing relationships with primary care providers/patient-centered medical homes.
  • Document and follow-up on resident medical appointments.
  • Responsible for outcomes reporting and quality reporting
  • Support the digitization of historical client records by scanning and integrating documentation into InSync.
  • Perform all other duties as assigned
