Requirements
- Active Registered Nurse (RN) license in the state of Wisconsin
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
Nice To Haves
- Previous healthcare experience preferred, but not required
Responsibilities
- Conduct in-person patient assessments in their homes
- Develop individualized care plans for caregivers to follow
- Maintain accurate documentation within our online charting system
- Communicate regularly with the Eau Claire office for updates and support
- Manage aspects of patient care, including case coordination
- Travel up to one hour to complete patient assessments
