Requirements
- Current and active RN license in New Jersey (required)
- 1+ years of nursing experience preferred
- Strong interpersonal and communication skills
Responsibilities
- Monitor resident health and wellness through assessments, observation, and timely follow-up
- Supervise and support Resident Care Assistants to ensure quality care and service delivery
- Assist the Director of Resident Life (DRL) with coordinating resident care and support services
- Complete assessments, reassessments, and individualized service plans in collaboration with residents, families, and care team members
- Coordinate resident transitions, appointments, transportation, referrals, and related documentation
- Oversee medication management, wellness services, and compliance with state regulations and community standards
- Assess, document, and follow up on resident incidents, accidents, and changes in condition
- Partner with residents, families, and interdisciplinary team members to promote resident well-being, safety, and quality of life
