About The Position

Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support—partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential. We're looking for a registered nurse to support adults in their homes and communities. This is a hybrid, per diem position: most of the work, documentation, coordination, and care planning is done remotely. In-person time is reserved for what truly requires it: patient evaluations, assessments, and CHHA determinations. All clinicians are expected to participate in the firm's accreditation activities and surveys to maintain our quality standards.

Requirements

  • Active NJ Registered Nurse license in good standing.
  • Mid-level clinical experience; home care or I/DD experience preferred.
  • Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits.
  • Strong documentation habits and clear communication with patients, families, and physicians.

Responsibilities

  • Conduct comprehensive health and psychosocial evaluations, and monitor each patient's condition.
  • Coordinate total patient care—health promotion, prevention, service coordination, and teaching.
  • Determine the need for and frequency of CHHA services based on patient and home assessments.
  • Develop and implement individualized nursing care plans; evaluate their effectiveness on an ongoing basis.
  • Complete admission, transfer, re-certification, resumption-of-care, and discharge documentation.
  • Submit clinical and progress notes weekly, plus a daily tally of patient visits.
  • Consult with attending physicians on care plan changes; loop in your supervisor as needed.
  • Present records to the Clinical Record Review Committee and participate in case conferences.
  • Explain nursing and firm services to patients and families as part of care planning.
  • Participate in firm accreditation activities and surveys.
