About The Position

The Primary Health Care (PHC) Registered Nurse (RN) is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The PHC RN promotes quality health care outcomes working within established standards for case management practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department.

Requirements

• Valid Class 5 driver's license
• Bachelor Degree in Nursing
• Leadership skills
• Critical thinking skills
• Communication skills
• Public relations skills
• Organizational skills
• Minimum 2 years nursing experience in primary care, public/community health
• Experience in teaching adults, facilitation skills, and leading group activities
• Ability to take initiative
• Demonstrated clinical competency
• Knowledge in chronic disease prevention and management
• Demonstrated ability to work in a multidisciplinary setting
• Ability to adapt to and implement change
• Ability to be self-directed
• Advance computer skills, including Microsoft Office (Excel, Word and Access)
• Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

Nice To Haves

• Master level preferred
• Experience in case management is an asset
• An equivalent combination of education and experience may be considered
